Job overview

Main duties of the job

As a Cancer Pathway Navigator you will ensure concise and daily tracking of each step in a patient pathway, expediting appointments/visits ensuring patients are not delayed at any point in their pathway.

You will actively participate in PTL meetings and also work collaboratively with other regional Trusts to support the smooth transition of patients referred in from tertiary centres.

The post holder will act as patient advocate and facilitator to overcome barriers to timely access to Lung Cancer services, utilising interpersonal skills and resources across the health service, third sector and local authorities.

Close liaison with the wider multidisciplinary team will be essential to ensuring investigations are booked, reports are available and staging is complete in time for follow up appointments.

Detailed job description and main responsibilities

- Proactively monitor patients’ progress along their pathway on a daily basis, highlighting problems to the management team and escalating where necessary according to local protocols.
- Facilitate and coordinate triage meetings and ensure that actions arising are completed.
- Attend local, regional and specialist MDT's to understand next steps in the patient's pathway, working with the clinical team to ensure the patient is promptly fully informed.
- Build and maintain effective working relationships with the wider MDT in pursuit of delivery of care within defined cancer wait time targets and NOLCP timed milestones.
- Actively identify situations where dates of diagnostic tests and treatment may not be scheduled appropriately, exercising own judgement and be responsible for ensuring appointments/procedures are expedited as necessary with guidance from line manager.
- Work closely with the administrative services across sites (including booking team), diagnostic services and clinical team to ensure next actions are regularly updated and completed.
- Develop and maintain knowledge of the lung cancer pathway provision across all subspecialties and have detailed understanding of steps within patient pathway and the associated flows of information.

Essential criteria
- NVQ Level 3 in Admin, Customer Services or a Health or Social care qualification or equivalent experience
- 4 GCSE’s including Maths and English at Grade C or above (or equivalent)

Experience

Essential criteria
- Recent experience of working within an NHS setting.
- Significant admin experience including use of clinical systems.

Desirable criteria
- Knowledge of working in a patient focused environment.

Communication and relationship skills

Essential criteria
- Excellent communication skills (written, verbal and interpersonal skills).
- Excellent listening and negotiation skills.
- Excellent customer care practice, being polite, sympathetic, patient, professional and diplomatic.
