Manage and support the Harm Free Care Falls Strategy for NUH.

To work closely with other Matrons to ensure collaborative working and achievement of Trust objectives related to the Harm Free Care Strategy for Falls, supporting a key Trust Quality Priority of the reduction of Falls.

To work closely with Divisional Teams, Pathway Lead Clinicians, Patients, Carers and the wider Public to examine the patient’s journey and redesign the process in collaboration with other members of the multi-professional team in order to deliver an improved patient experience and modernisation of services.

In conjunction with the Head of Safeguarding and Harm Free care, supporting the delivery of a balanced budget, taking appropriate remedial action when required, and ensuring adherence to Trust Financial Policies and Procedures.

Identify and develop systems for collection and analysis of information relating to the Harm Free Care Strategy for the Falls Strategy and the Service Manage workload, resources, equipment and supplies to achieve the provision of high quality services.

Contribute to annual planning and local service delivery plans.

Working with a vision of “delivering excellence in all everything we do,” the Matron will work with the** **Chief Nurse, Deputy Chief Nurse, Head of Safeguarding and the Patient Safety Team to provide senior leadership in delivering and supporting the management of Falls, with a particular focus on the prevention of moderate and severe harm falls across the organisation to provide the best possible specialist care for patients with these vulnerabilities. The Matron will be a strong Clinical Leader, easily identifiable, highly visible and accessible to patients, carers and staff. He/she/they will have a particular responsibility to support and progress the management of Falls delivering frontline professional and operational advice.

He/she/they will have a particular responsibility for overseeing external and internal reporting of incidence and prevalence, working closely with the Adult Lead for Safeguarding to ensure the organisations meets a key statutory responsibilities in relation to the management of Falls.

The post holder will work closely with other members of the Corporate Nursing and Divisional teams to ensure that a seamless service is provided to patients and will be involved in the ongoing development of specialist service provision. The post holder will be required to:

In conjunction with the Head of Safeguarding, lead and implement key objectives for the Trust associated with Harm Free Care for Falls.

Be an effective leader and role model and to support the implementation of both clinical and non‑clinical governance.

Provide the ongoing development and co-ordinations of a Trust-wide staff training programme for Falls prevention.

To work closely with the Patient Safety Team to develop flexible and innovative approaches to implement evidence based care for Falls management and play a key role in the implement of the Patient Safety Incident Response Framework (PSIRF).

Detailed job description and main responsibilities

**Governance and Risk**

Ensure that all untoward incidents and near misses are reported by staff within the area of responsibility in accordance with the Trust policy, and that appropriate mechanisms are put in place to investigate serious clinical incidents under the direction and guidance of the Risk Management Department, Safeguarding Leads, Harm Free team, and Patient Safety Teams. Following which appropriate action plans are developed and monitored, and lessons are learned and disseminated.

To contribute to the long-term strategies of the Service, including service reviews and developments and work closely with the Patient Safety Team in the development of the Safety-II approach at NUH.

Implement systems and processes that create and maintain a culture which actively promotes staffs’ contribution to the development and delivery of clinical governance and risk management objectives.

Support investigations and resolution of actual and potential complaints at a local level and work closely with the Divisional Nurse and ward/unit /department leaders to ensure that learning takes place as a result of
