The Home First and Reablement service in Gloucestershire is gaining strength and recognition for its ability to enhance independent living for people following their discharge home from a hospital environment. The service utilises the skills and experience of a multidisciplinary team, including therapists, nurses, Home First and Reablement support workers and the wider community to deliver person-centred care and enable people to maximise their opportunities. We use a strengths-based approach to support people to optimise their potential and find solutions to manage their situations and remain living well for as long as possible

We are looking for a Practice and Learning Facilitator to join our team and support the training and development needs of the Home First and Reablement staff within the service. Working with the support of the Trust Learning and Development team, this is an exciting opportunity for you to use your clinical skills and experience of working within multidisciplinary teams to nurture and develop others. Our vision is to maintain a competent workforce, providing high quality, evidence-based interventions that meet the needs of our local population, and the post holder will be instrumental in achieving this

Working alongside the Reablement lead and professional colleagues the post holder will devise and deliver training opportunities through a range of mediums to support induction programmes and on-going continuous professional development.

The successful post holder will have experience in training, and work place staff performance management, utilising a range of metrics and IT capabilities. Applicants will be strong communicators across a range of diverse staff groups and have a proven record of continuous professional development. A UK driving license and access to a car is essential, and agile working will enable you to access all the localities across the County
