The CHC delivery team in NNICB require an experienced Registered Children's Nurse to take on the role of Clinical Case Manager. 1. To oversee children and young people in receipt of children’s continuing care by ensuring the commissioned care is being delivered and is clinically evidenced -. This will include clinical review of CYP packages of care to ensure they are cost effective and delivering the outcomes intended for the CYP, with personal health budgets (PHBs).

2. To provide expert support and advice to the CYPCC service and the internal ICB CHC team - the post holder will work closely with the ICB’s commissioned service provider to oversee the CYPCC patients on their caseload.

3. Lead the CYPCC Panel process - chairing the Panel meetings, approving proposed packages of care via the ICB’s governance process, overseeing the panel administration and recording of approved funding on the ICB database.

4. Providing project support - to lead on the identified CYPCC workstreams within the ICB’s annual plan and complete weekly Highlight reports.
