**BASED**:Holloway, Islington N7. Our staff are able to work from home until Covid-19 restrictions ease and it is safe to return to in person working.

**HOURS**:21 hours per week

**SALARY**:NCJ Scale 4 point 13 - 16, £29,522 - £31,144 pro rata (actual salary £17,221 - £18,167).

**JOB SUMMARY**:
The Health Connector will play a crucial role in our charity's efforts to provide advocacy, support, and wellbeing services to our clients. This role is responsible for ensuring that clients have access to the necessary services, support, and activities to enhance their overall physical and mental wellbeing. The Health Connector will have a good understanding of the health and welfare landscape and experience of working with clients from diverse backgrounds, including conducting needs assessments and providing advocacy and support.

The Health Connector role sits within the Advocacy Service at Manor Gardens. Together, this team is responsible for our front door and open access services, including the oversight of our universal referral service. The post holder will work closely with the Wellbeing Team and the Co-production Manager to design and deliver a programme of community outreach, engagement, events and activities designed to improve the health and wellbeing of Islington residents. Knowledge of a second language would be an advantage.

**LIAISON WITH**:Advocacy and Wellbeing team staff, MGWT volunteers, sessional staff as required, relevant external organisations and stakeholders.

We work with people to take action to change their lives for the better. Our services promote mental and physical health and wellbeing and increase social inclusion.

Our serviceshelp people manage their own health and wellbeing and maximise their independence. Our services are diverse, reflecting the communities we work with, and include bilingual support for refugees and migrants, a food co-op for people struggling with food insecurity and a range of health and wellbeing activities, from mental health support groups, LGBTQ+ groups, art and exercise groups.

**ROLE OVERVIEW & PURPOSE**

**Direct Support (30%)**
- Work with key local stakeholders to ensure that 60 clients per year from various local communities have access to the service.
- Carry out needs assessments and draw up action plans for each client to enable them to access services to improve their health and wellbeing.
- Support clients in seeing through their action plans, including accessing advice or services, working alongside MGWT colleagues to provide further support, and ensuring successful referral to support from partner organisations where relevant.
- When necessary provide support to isolated clients through house calls or by assigning buddies.
- Facilitate better access to targeted support services, including transport and PIPs, through advocacy, home assessments and buddy programmes.
- Ensure client support provided is in line with MGWT policies and procedures, in particular MGWT Safeguarding Policies and Procedures.
- Ensure the continual improvement of our offer, researching and promoting best practice from across the sector, and keeping up to date with current services offered by our partners.

**Groups and activities (30%)**
- Work with the Coproduction Manager, Volunteer and Activities Coordinator, Advocacy Manager and our communities themselves to design and deliver a variety of groups and activities to improve health and wellbeing. This will include:

- Working with colleagues and communities to understand key needs and requests, and come up with new ideas.
- Work with key stakeholders from health, social care and the VCS to develop and deliver services to improve the physical and mental health and wellbeing of residents experiencing health inequalities.
- Planning and administering the event management process, including coordinating the involvement of other staff, volunteers and external facilitators
- Collecting feedback and ideas to continually improve on the activities we are running

**Volunteer involvement (10%)**
- Work with an active team of volunteers who support our front door services and group and activities, whilst also positively impacting their own personal and professional development.

**Data and impact (10%)**
- Ensure accurate data is collected and recorded effectively in our CRM system, and contribute to the reporting and evaluation requirements of the service, with support from the Advocacy Manager.
