We have an opportunity for a Registered Assessment Practitioner to join the EIP Team.

The post holder will work as a member of the EIP Service, providing a high-quality care service to 14-65year old at risk of developing psychosis.

This role is to participate in initial assessment using a Comprehensive Assessment of At-Risk Mental State Tool (CAARMS) to determine appropriateness for EIP service. The practitioner will work alongside the referring teams to identify alternative services as appropriate to meet the persons needs.

The post holder will be involved in a detailed assessment period if required exploring the bio-psycho-social elements of the individual. The assessment period will also include engaging with carers and support network, alongside initial psychoeducation surrounding the EIP ethos and culture. You will also support the team with other duties where and when service need is required.

As an assessment practitioner within the service, you will lead on specialist assessments using the CAARMS (Comprehensive Assessment of At-Risk Mental State) tool, alongside providing specialised and tailored interventions as part of extended assessment periods. Your day-to-day duties will include:
Initial screening and leading on assessments using the CAARMS tool.

Liaison and collaborative working with referrers from all sectors including primary care, social care and inpatient teams to provide specialist assessment and advice.

To work as part of our integrated MDT, and to participate in a clinical decision-making process to determine the most appropriate pathway for all service users who are under the referred to the service.

Travel across Lincolnshire to support the needs of service users, their family members, and carers.

Provide support, advice and guidance to other members of staff.

Medication management and intervention delivery.

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.

To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.

To ensure the clinical caseload and clinical practice of nursing members of the EIP Team is of the highest standard of clinical care.

To manage case-load and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.

Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behavior.

Plan, implement, review and improve interventions to meet people’s identified needs and manage their inherent risk.

To assess carers’ and family’s needs and develop, implement and review programs of support for cares and families.

Protect people from abuse, neglect and harm.

Enable individuals to develop independent living skills and live in the community.

To work collaboratively and promote effective working relationships with members of the multi¬disciplinary team, ensuring effective and appropriate clinical decision-making, both within the EIP Team and with external agencies.

To maintain accurate and timely clinical records and to co-ordinate and monitor those of the team.

To adhere to NMC, HCPC and other regulator HCPC codes of professional conduct and ethics, plus associated legislation.
