Job summary

An exciting opportunity has arisen for a highlymotivated and enthusiastic Nurse Practitioner to join a friendly, supportive anddedicated team. We are looking to appoint an Advanced Nurse Practitioner/AdvancedClinical Practitioner who can work effectively within our clinical team, toundertake patient assessment, diagnosis and clinical management. We would consider a newly qualified ANP/ACPand would provide ongoing mentorship for learning needs and support.

The vacancy is for 6 to 8 sessions.

Main duties of the job

Working as an independent prescriber who will be ableto diagnose and treat patients presenting with acute problems and chronicdiseases, via face to face, telephone and online consultations.

Thecandidate will need to prioritise and triage the needs of patients accordingly,making any necessary referrals for investigations in the appropriate manner

Be self-motivated, with good organisational andpresentation skills. Have the ability to work in ever changing, demandingsituations.

Bassingham Surgery is a busy three partner, rural,forward thinking & innovative, dispensing practice, serving approx. 7,000patients providing an excellent level of care and service. We employ 1 salariedGP, 2 Advanced Nurse Practitioners, 2 Practice Nurse and 2 HCAs with a fullcomplement of support staff, administration, reception & dispensary.

Our staff are friendly, hardworking, caringindividuals and many are long term employees, who work as a close, sociableteam, enjoying the challenges of primary care.

We are part of South Lincoln Healthcare PCNalongside six other practices, employing additional roles, including aHomeHealth Team who visit our local nursing home; Occupational Therapists,First Contact Physios, Clinical Pharmacy Team, Health & Wellbeing Coach,Mental Health Practitioner and Care Co-ordinators; this aspect of Primary is constantly developing.

Job responsibilities

The Nurse Practitioner/Clinical Practitioner willwork autonomously within their professional scope of practice providing acutecare for patients via face to face, telephone and online consultations and whenrequired, home visits (these are very occasional). The ANP/ACP will need to provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care, making necessary referrals for investigationsin the appropriate manner.

They will demonstrate safe, clinical decision-makingand expert care, including assessment and diagnostic skills, for patientswithin the general practice. They willdemonstrate critical thinking in the clinical decision-making process and willwork collaboratively with the practice team to meet the needs of patients andsupport the delivery of policies and procedures.
