Following internal promotions and progression, we are delighted to offer an exciting opportunity for experienced enthusiastic clinician to join the patient flow team. The patient flow team is a small and dynamic team. Working directly with inpatient teams and community teams to support discharge from mental health inpatient services. Working collaboratively with system partners to ensure the safe discharge of patients to meet their current needs.

**The Role requires**: To have oversight of cases that are not currently allocated to the Care Co-ordinator and to assure that discharge planning is progressed until allocation happens. To support the optimisation of bed utilisation and reduce the length of stay and delayed transfers of care within mental health, ensuring the timely and safe transfer of care is undertaken. To manage the necessary information via internal systems and ensure that all data is live and accurately recorded. Liaison with community Team Managers within the CMHT to ensure that referrals are prioritised and allocated for discharge planning purposes at the point of admission within the first 5 days from admission.

To ensure that the Care Co-ordinators are in attendance for necessary discharge planning starting from the point of admission. Where this is not happening the post holder will escalate and progress allocation requirements to the identified community lead. Provide regular updates to the S&Q lead for length of stay and delayed transfers of care. To provide the operation leads with the necessary weekly reports to support the DTOC LOS and Bronze escalation meetings.

To assist the team to update and report to the identified contracting team and support the reporting mechanisms. To highlight/escalate to the Pathway/Service Manager any issues in relation to delays. For Further information please see JD and Person Spec.
