2. Role Context Your Healthcare (YH) is a not for profit making organisation, one of the families of organisations working in partnership to deliver holistic provision. Providing a wide range of public funded health and social care services for children, adults and families and a full range of enabling services. It employs circa 700 staff based across a number of YH, local authority and NHS owned sites.

YH forms part of a public sector 'mutuals' collaborative called Albion Care Alliance CIC (known as Albion) which is currently comprised of three mutual or co-owned organisations. These are Provide, Medway Community Healthcare (MCH CIC and Your Healthcare CIC. Albion has over 4,000 staff delivering health and social care services and a combined total turnover of circa £160 million. Albion aims to collaborate to offer benefits of scale to partner organisations.

Albion, through the facility of Albion Outlook Ltd offers the partner organisations an ability to invest in product development relevant to the businesses and to launch initiatives outside of the usual range of one of the individual partner organisations if they were to act alone. 3. Job Purpose and Key Result Areas Work in collaboration with local GP services, local adult social care services, employers and colleges to deliver a coordinated and high-quality social prescribing service. Supporting autistic adults to improve their health and wellbeing and to facilitate engagement with local community based support, opportunities and activities.

To spend time with autistic people to help them to focus on what matters to me and connect them to community groups and statutory services for both practical and emotional support. Manage a caseload of clients through assessment of need to onward referral. To provide ongoing support for a short-term timeframe to promote engagement with identified services and achievement of goals. Contribute to the development of the service and participate in support, supervision and training as required.

To contribute to raising awareness of the needs of autistic adults by preparing and delivering training to others. To facilitate peer support groups for autistic people and to review the impact of the social prescribing role in line with the service evaluation project aims. Key relationships include: The NHS Integrated Care Board, Adult Social Care, childrens services (Achieving for Children), local GP Practices within the specific primary care network, local colleges, employers, social and health care services and the diverse range of voluntary sector organisations. Main activities and responsibilities To contribute to the Adult Autism Service by: Maintaining a clinical case load Supporting people to establish their personal goals and develop support plans to meet these Review the impact of this support on each persons quality of life and community engagement Prepare and deliver training to increase the awareness of the needs of people with autistic people across services within Kingston and Richmond To prepare and facilitate peer support groups for autistic people and their families.

Positively engaging in supervision and using this to reflect on clinical practice and on the organisational ethos. Develop trusting relationships with people and families that have been referred to the service, giving them time to focus on what matters to them and providing them with personalised help to take control of their health and wellbeing, live independently. Undertake needs assessments in the community. Support a caseload of clients for whom social prescribing might offer improved outcomes.

Undertake regular case meetings with senior management, stakeholders to discuss Social Prescriber sessions. Through regular networking, proactively identify people who would benefit from this type of help and manage and regularly review the caseload to accommodate urgent referrals for support as required. To introduce and be in contact with multi-disciplinary teams across the Primary Care Networks member practices. Following an agreed period of support (with in-built flexibility), refer clients on to the voluntary sector or local community support services where appropriate.

Make recommendations on where a social prescribing approach might support the client better or complement existing clinical interventions and improve outcomes.
