Main duties of the job
Youwill work with our community nursing teams, local practice nurses, inpatientward and local care homes to support them in providing effective,patient-centred wound care, offering specialist advice and support for peoplewith a variety of wounds. The successfulcandidate must possess relevant knowledge, skills and experience in pressureulcer, leg ulcer and complex wound management.Knowledge of national guidance around pressure ulcer reporting andmanagement, and leg ulcer management is essential.
Provisionof wound care training is an integral part of the role and therefore previousexperience in training and/or mentorship is highly desirable.
Youwill need to be well organised and able to plan your own workload, haveexcellent written and verbal communication skills, and be able to workautonomously making clinical decisions and providing guidance to otherclinicians.

2. Job Purpose
To provide evidence-based and clinically effective care to patients with tissue viability needs To act as an expert resource for community staff in the area of tissue viability To support local care homes in the assessment and management of residents with tissue viability needs To work as a member of the multi-disciplinary team, developing and maintaining effective working relationships with other agencies including acute, primary and voluntary sectors To support the delivery of educational programmes within Your Healthcare To contribute to the quality agenda within Your Healthcare with particular focus on reducing pressure ulcer incidence3. The post holder will support the care of people with tissue viability needs registered in the borough of Kingston upon Thames in their own homes, care homes, local GP practices and YH community in-patient unit at Teddington Memorial Hospital
The post holder will provide high quality skilled nursing care, contributing to the on- going assessment, planning, implementation and evaluation of care
The post holder will support the Tissue Viability Nurse Specialist in clinical audit, service improvements, delivery of education and initiatives to ensure continuous quality improvement
The post holder will be required to travel between sites
4. Key Result Areas
Professional/Clinical
To manage referrals into the tissue viability service, prioritising according to clinical need To be responsible for assessing, developing, implementing and evaluating holistic care for patients with tissue viability issues in partnership with others To provide sound clinical advice to patients and clinicians in the management of their wounds To support the tissue viability specialist nurse in measuring outcomes of care delivered within the leg ulcer and tissue viability service through clinical audit and other benchmarking indicators To ensure standards, policies and protocols relevant to tissue viability are current and adhered to To participate in pressure ulcer monitoring via Datix incident reports, and Safeguarding To participate in the South West London wound formulary process and support adherence To cover for the Tissue Viability Nurse Specialist in their absenceCommunication
Ensure comprehensive, accurate and confidential patient records are maintained using the electronic care records system and other documentation approved by the organisation Deal with complex, sensitive information using sensitivity and empathy Observe, document and report to the specialist nurses, GP or consultant, as necessary, changes in the patients condition, be that physical, mental and/or social Establish and maintain effective communication with patients, colleagues, statutory and voluntary servicesDevelopment and training
Participate in planning and delivering programmes of education for all members of staff and students both internally and externally To develop educational and training resources for staff and patients Be responsible for identifying personal learning needs and maintenance of own competencies within area of specialism Person Specification

Clear understanding of current tissue viability best practice and national guidelines Knowledge of current professional issues in wound care, particularly around pressure ulcer reporting and management Evidence of continuing professional development

Knowledge and experience of clinical audit, research and quality improvement

Previous relevant experience in wound care/tissue viability in community setting Ability to assess patients with complex health and social needs

Registered Nurse Evidence of post-registration study in wound care

1st Level Degree in relevant health care V100/150 or V300 prescribing qualification Teaching qualification

Able to communicate effectively, both verbally and in writing Teaching and presentation skills Confidence in use of larvae therapy and negative pressure wound therapy Confidence in establishing rapport and trust with patients, carers, family and staff groups Ability to make confident professional judgements and care management decisions without direct supervision Computer skills, ability to use word, excel, power-point Ability to meet deadlines as required Good organisational and planning skills
