Tissue Viability Nurse Specialist

We are seeking a highly skilled and experienced Tissue Viability Nurse to join our team at Your Healthcare CIC. As a Tissue Viability Nurse, you will play a crucial role in providing expert care to patients with tissue viability needs, working closely with community staff, local care homes, and other agencies to ensure effective wound management.
Main Duties- Provide evidence-based and clinically effective care to patients with tissue viability needs.
- Act as an expert resource for community staff in the area of tissue viability.
- Support local care homes in the assessment and management of residents with tissue viability needs.
- Work as a member of the multi-disciplinary team, developing and maintaining effective working relationships with other agencies.
- Support the delivery of educational programmes within Your Healthcare.
- Contribute to the quality agenda within Your Healthcare, with a focus on reducing pressure ulcer incidence.
- Manage referrals into the tissue viability service, prioritizing according to clinical need.
- Assess, develop, implement, and evaluate holistic care for patients with tissue viability issues.
- Provide sound clinical advice to patients and clinicians in the management of their wounds.
- Support the tissue viability specialist nurse in measuring outcomes of care delivered within the leg ulcer and tissue viability service.
- Ensure standards, policies, and protocols relevant to tissue viability are current and adhered to.
- Clear understanding of current tissue viability best practice and national guidelines.
- Knowledge of current professional issues in wound care, particularly around pressure ulcer reporting and management.
- Evidence of continuing professional development.
