Rapid Response Practitioner

2. Job Purpose Your Healthcare (YH) Rapid Response Team is an integral part of the multi-disciplinary Urgent Community Response Service. The team responds within 2 hours to adults in their own home, who are experiencing an acute or sudden deterioration in their health and wellbeing, with the aim of preventing an unnecessary hospital admission. The team provides advanced clinical assessment, diagnosis, management plans and integrated working with health and social care services to support people at home.
3. Key Result Areas 3.1. Clinical Assess and triage referrals to ensure they meet the criteria for urgent community response (UCR) intervention Use clinical reasoning and physical assessment to assess, diagnose and treat the needs of patients, presenting with a deterioration in health and wellbeing who may be at risk of a hospital admission Formulate and implement a comprehensive evidence-based plan of care with the patient/relative/carers, promoting choice and empowerment of the patient Provide expert clinical knowledge, skills, advice and health promotion to patients/ carers/relatives to support them in their own home To provide/facilitate a range of appropriate care at home including but not limited to acute respiratory exacerbations, urinary tract infections, pain management and bowel management as required Act where appropriate as a Non
- Medical Prescriber, taking responsibility for prescribing based on efficacy, safety and cost Evaluate treatments plans for patients with acute and/or complex needs and amend plans in response to patient treatment. Use appropriate assessment tools such as NEWS2 to assess for deterioration, prioritise health problems and escalate effective emergency care or urgent care as appropriate Initiate appropriate and timely consultation and/or referral with appropriate medical professional when the problem exceeds own professional scope and practice Work collaboratively with UCR therapists, community nursing services, allied health professionals, General Practitioners, adult social care, 111, ambulance service and relevant health and social care partners 3.2.
Service Delivery To contribute to day to day delivery of the Rapid Response team including appropriate delegation and organisation of workload within the team, absence and performance management. To practice within the framework provided by the YHC Policies, Procedures and Guidelines and those laid down by professional regulatory body To contribute to the delivery of the Rapid Response service Participate in the development and integration of care pathways, policies and procedures, which will influence service delivery and practice. To record accurate legible health records To accurately record, review and report on service performance 3.3. Management To have an awareness of budgetary constraints and resource issues when delivering care.
To support safe and efficient staffing levels through the planning of appropriate and flexible duty rotas. To undertake supervision and performance development reviews with team members To participate in the recruitment and induction of new staff. To plan, coordinate and contribute to the induction, training and education of staff. To teach, mentor and assess pre and post registration nursing and allied health professional students To provide expert clinical advice and support to team members 3.4.
Quality To accept professional responsibility for own work and work delegated to others. To practice within the professional bodys codes of conduct at all times To be up to date with current practices and procedures and promote evidence-based practice. To participate in a supervision and appraisal process and ensure the development and maintenance of a personal development plan which contributes to team and organisational objectives To provide fair and equitable opportunities for all staff promoting a culture of equality, diversity and inclusivity To practise and promote safe service effectiveness and clinical governance To undertake the review and investigation of incidents as delegated by the line manager per the organisations internal process and policy To promote and deliver the CQC quality framework in practice To actively participate in audit and standard setting To support the evaluation of the effectiveness of care within the team. To contribute to policy development and changes in practice including those that may impact on other areas To keep up to date with current research and national guidelines to recommend changes to service delivery and/or clinical practice
