2. Job Purpose To respond to urgent community referrals within 2 hours to those adults who are experiencing a sudden deterioration in health or well
- being and require crisis intervention to prevent an unnecessary hospital admission To complete a comprehensive holistic assessment and formulate a management plan To develop skills in autonomous practice including physical assessment, clinical reasoning and differential diagnosis To work collaboratively with urgent community response (UCR) therapists and multidisciplinary teams in the management of complex health and social care issues To provide supervision and support to students and junior staff as appropriate To contribute to the rapid response/UCR service delivery ensuring a responsive, high quality and effective service To record accurate legible health records 4. Key Result Areas Clinical To visit patients in their home and perform a comprehensive assessment To assess and triage referrals to ensure they meet the criteria for Rapid Response/UCR intervention To formulate comprehensive and concordant plan of care with the patient and/or their carers, promoting choice and empowerment of the patient To review patients response to treatment and make reasonable adjustments following discussions with senior members of the rapid response team To understand the implications of clinical diagnosis in order to provide appropriate health promotion advice to patients and/or carers To develop and implement physical assessment and clinical decision-making skills, to enable patients who are acutely unwell or in a crisis to stay at home, avoiding unnecessary hospital admission To develop skills in formulating differential diagnosis and treatment plans through training and collaborative working with Senior Rapid Response Practitioners To provide/facilitate a range of appropriate care at home including but not limited to acute respiratory exacerbations, urinary tract infections, acute infections, acute confusion, pain management, gastrointestinal disturbances and management as required To carry out a range of clinical procedures such as, but not limited to, venepuncture, bowel management, intravenous therapy, and wound dressings. 4.2.
Service Delivery To contribute to the day to day delivery of the rapid response team including appropriate delegation and organisation of workload within the team To accurately record activity data and contribute to meeting the teams key performance indicators. To contribute to the delivery of the rapid response/urgent community response service To participate and contribute to team meetings To supervise junior staff/students where appropriate To practice within the framework provided by the YHC Policies, Procedures and Guidelines and those laid down by professional regulatory body 4.3. Management To respond to the delegation of duties by the Team Leads To participate in the recruitment and induction of new staff as delegated by the Team Leads To plan, coordinate and contribute to the induction, training and education of staff. To teach, mentor and assess pre and post registration nursing and allied health professional students To provide advice and support to YH community and allied health professionals as appropriate 4.4.
Quality To accept professional responsibility for own work and work delegated to others To practice within the professional bodys code of conduct at all times To be up to date with current practices and procedures and promote evidence-based practice To participate in the supervision and appraisal process and ensure the development and maintenance of a personal development plan which contributes to team and organisational objectives To provide fair and equitable opportunities for all staff promoting a culture of equality, diversity and inclusivity To demonstrate understanding of clinical governance and risk management To promote and practice within the CQC quality framework To actively participate in audit and standard setting To support the evaluation of the effectiveness of care within the team To contribute to policy development and changes in practice including those that may impact on other areas To keep up to date with current research and national guidelines to recommend changes to service delivery and/or clinical practice
