**The Role: One Community delivers a range of options which benefit the residents of the borough of Eastleigh. This person will coordinate the ‘Getting Together Project’ which brings together a range of activities that reduce social isolation. They will need to be confident, self-motivating, positive and energetic, with an eye for detail and, excellent organisational and communication skills.**

**Summary-**
- To work in partnership with sheltered home providers to provide activity sessions to their residents that support their wellbeing and promote community engagement.
- To plan and deliver ‘Trips’ for the residents of Eastleigh borough as referred, to support their wellbeing and reduce social isolation.
- To deliver the reducing isolation project (previously the Back to the Future project) in the borough of Eastleigh.
- Initially to provide 2 sessions of activities per week within sheltered accommodation.
- To promote and organise the development of further sessions in different locations.
- To organise ‘trips out’ to other locations and venues with both One Community clients and those referred from other organisations
- To monitor, take referrals and support volunteers to support residents/clients to engage in their community and/or communicate with others, as agreed.
- To recruit, support and supervise volunteers to provide assistance across the project.
- To consult with clients to ensure the delivery of activities meets their interests, promotes choice and are inclusive, and to give due regard to the needs of carers as appropriate.
- Organise and engage entertainment and deliver the activities.
- To prepare drinks and light snacks
- To be responsible for safeguarding the attendees, ensure effective risk assessments are in place and, appropriate to those activities.
- To provide cover across all the One Community Day Centres and Home Services as required
- To gather and record information and outcome measurements, to be able to effectively provide monitoring reports for funders

**Other**
- To ensure compliance with One Community policies and procedures
- To effectively use monitoring, evaluation, and reporting systems
- To ensure that issues raised by clients and their carers/family members are treated with respect, sensitivity and that confidentiality is maintained

**GENERAL**
- To be largely self-sufficient in terms of the post’s administrative work

- Deal with members of the public, users of the service and other professionals in an appropriate and sensitive manner and to communicate effectively with them verbally and in writing
- In all aspects of handling personal information all employees are required to work to maintain confidentiality and data protection
- To participate in One Community meetings and appraisal sessions as appropriate

**CONDITIONS OF SERVICE**

**HOURS**: 18 hours per week
- Days and 6 hours of the role are on Tuesday and Thursday for delivery of sessions initially.

Time off in lieu is available when the post-holder may on occasions, be required to work additional hours to fulfil the needs of the service.

**Location**:
The post holder will be based at our Head Office at 12 Romsey Road, Eastleigh. However due to the nature of the role will be required to work across the Borough of Eastleigh at different locations.

**Salary**: FTE £23,896.08 (Actual £11,625.12)
