Greater Peterborough Network was set up in 2015 by local GPs to deliver high quality patient care in the community. We are an NHS Healthcare Provider owned and run by the GP Practices within the Greater Peterborough area. We work for and with our member Practices developing at scale community services to support the health and well-being of the Greater Peterborough population. This role forms part of GPN’s workforce strategy to recruit and develop a centralised clinical team that will work across a diverse portfolio of projects to address health inequalities and support the local health and care system deliver truly integrated care to our population.

This role will be part of a new, innovative city-wide primary care Community Urgent Response Team delivering virtual and face to face clinical capacity to support patients across Huntingdon who are acutely unwell and unable to get to their GP Practice. Working as part of a multi professional home visiting team the role will work autonomously using clinical assessment and treatment skills to provide responsive, on the day clinical capacity to patients living in Huntingdon.

Our vision is to develop a multi-disciplinary community urgent response service to support some of the City’s most vulnerable patients, providing responsive capacity to our GP Practices and local health and care system. Over the last 2 years GPN have developed a Nurse led, HCA home visiting service to support practices to manage their housebound patient population. More recently we have developed a team that supports the Ambulance Service to refer patients who, with support, are able to remain at home. This service has reduced conveyances and enabled paramedics to reduce the time spent in a patient home. GPN are now seeking to build on this success and expand our team and scope of service. This will be non planned, reactive work to see patients who have been in contact with the Ambulance Service.

The applicant is required to on occasion work in the extended access HUB to ensure clinical skills are upheld.

This service is 8:00am - 20:00pm, 7 days a week.

Main duties of the job

- Support people to improve and maintain their mental, physical, behavioural health and wellbeing.
- Actively be involved in the prevention of and protection against disease and ill health.
- Engage in the public health, community development and in the reduction of health inequalities.
- Provide compassionate, safe and effective care and support to people in a range of care settings.
- To contribute to assessment, planning, implementing and evaluation of individual cases.
- To demonstrate sound knowledge of care and treatment in order to undertake non-complex nursing assessment/reassessment in various community settings under supervision of senior staff.
- To liaise with external agencies with regards to the development and implementation of care packages

Essential criteria

Registered with NMC Nursing Associate Evidence of continuing professional development

Full Drivers License

Experience of working within a team Passion for patient care Works well under pressure and enjoy change Experience in primary, community, or acute care
