Community Associate Practitioner

We are currently looking for a Band 4 Community Associate Practitioner who will be working as a member of the Out of Hours Countywide Nursing team, based in Huntingdon.
As a member of this team, you be trained and supervised by registered professionals and will be expected to follow a plan of care based on the specific needs of the service users. This is a great opportunity for an experienced, motivated, and enthusiastic individual to join our service providing care in patients’ own homes.
You will be responsible for the assessment, planning, implementation, and evaluation of nursing care in line with the agreed NMC standards specific to the role, and you will have a working knowledge of local, national policies and legislation that govern current service provision. Along with being responsible for identifying and contributing to the training needs of junior unregistered staff, and ensure that issues of equality, diversity and rights are dealt with in accordance with good practice and legislation.

Main duties of the job
Experience in community nursing.
- Educated to Registered Nursing Associate on the NMC Register.
- Experience in falls prevention work or willingness to gain skill.
- Experience in caring for and supporting adults and older people.
- Experience in assessing patients and formulating care plans independently and where appropriate, with support from RNs in the team.
- The ability to work flexibly and as part of a team, as well as individually.
- The ability to work flexibly working across both nursing and therapy.
- Good time management and the ability to manage individual workload.
