This is a great opportunity to work with young people admitted to paediatric wards within the Pennine Care footprint and who present with mental health needs or risks.
We are looking for individuals who are passionate about working with children and young people and who can work innovatively across traditional boundaries to ensure young people get the right care, from the right clinician/team, at the right time and in the right place; to improve outcomes and families/carers experience of mental health support.
We are seeking staff with a professional registration (Nursing, Social Work, Occupational Therapy) to work in a multi-disciplinary team offering assessment, support, signposting, and intervention in this new and developing pathway.
We are looking for staff with significant experience in either relevant community or inpatient settings with experience of assessment, formulation and managing complex cases and risk within a mental health. Working alongside senior clinical staff the post holder will respond rapidly to requests for assistance when a young person is presenting in crisis. The post holder will offer assessment and management within a clear and structured Risk Assessment and Risk Reduction framework and will refer on to specialist in-patient services as necessary or work collaboratively with other services involved to reduce the need for inpatient care. As a member of the multi-disciplinary team the service will intensively support young people and families.
Direct work with young people will take place in clinical environments, in paediatric wards and community settings.
