**_ Being a Support Worker isn’t something you do, it’s something you are._**

We are currently recruiting for a caring and compassionate Support Worker to join our vibrant supported living service in Acton. Our team is responsible for providing assistance to adults with learning disabilities and mental health needs. We do this by making sure that we provide the help they need to live life the way they want.

**Duties can include**:

- Promoting independence and wellbeing, ensuring that safeguarding is kept at the forefront of all the work we do
- Providing physical support which may include helping with household tasks and personal care.
- Providing emotional support for an individual and their families.
- Supporting and helping with health care needs, including routine appointments or administrating medication.
- Encouraging and supporting the development of personal skills through hobbies and interests.
- Teaching life skills, such as shopping, using public transport and paying for bills.
- Working with other healthcare professionals to ensure that all care needs meet the highest possible standards.
- Develop and implement individual Support Plans and carry out regular reviews
- Maintain a high standard of record keeping and ensure that appropriate administrative procedures are adhered to
- Carry out necessary health and safety checks as required
- Report repairs, renewals, health and safety concerns etc to appropriate colleagues in a timely manner
- Ensure that client confidentiality is maintained in accordance with organisational policy and the law
- Able to work alternate shift patterns including, days, evenings, and sleep-ins and weekends

**_ For this role you will be required to do evening, weekend work and sleep-ins so flexibility is essential._**
