About the Role

We are seeking a highly skilled and compassionate Support Worker to join our team at Chester Day Service. As a Support Worker, you will play a vital role in providing high-quality, enabling care to disabled adults, promoting independence, and supporting individuals to thrive and live their best lives.

Key Responsibilities
  • Implement person-centered support plans to promote independence and manage risks.
  • Develop flexible and engaging activity plans that promote autonomy and personal growth.
  • Support individuals in maintaining healthy lifestyles and managing their health needs.
  • Facilitate effective communication and maintain detailed, person-centered records.
  • Oversee medication processes and support individuals in self-administration.
  • Maintain a safe, clean-living environment and adhere to health and safety protocols.
