**JOB DESCRIPTION AND PERSON SPECIFICATION JOB TITLE**: HEALTH CARE ASSISTANT REPORTS TO: Nurse Team Lead HOURS: Part-time (Negotiable) Job Summary: To support the practice nursing team in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the patient population. The healthcare assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education, carrying out investigations and treatments in accordance with practice policy and protocols. Maintain a safe and effective practice at all times and bring any deficits in care to the attention of the Nursing Lead so that they can be addressed. Clinical responsibilities : Work closely with other health professionals in the practice to ensure high quality integrated care for the practice population Undertake diagnostic, health screening, health surveillance investigations as requested directed by the medical team or as part of an evidence based protocol Adhere to high standards of infection control measures when collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques Ensure that all clinical information is accurately and contemporaneously captured on EMIS Web, the practice clinical record system in accordance with agreed protocols and templates.

Assist in the assessment; surveillance and promotion of patients health and wellbeing Clinical duties : Phlebotomy obtaining venous samples for investigation on an acute and planned basis Ensure blood samples and specimens are recorded and ready for onward transportation for laboratory testing Undertake ECGs as requested and ensure that they are reviewed in a timely manner Chaperoning duties as requested by patients and colleagues Undertake observations, blood tests and lifestyle questioning as part of annual QoF reviews, Learning Disability reviews and NHS Health Checks Height, weight and BMI checks as requested. Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately as requested. Ambulatory and home blood pressure monitoring, including organising appointments, recording and saving the results. Event monitoring, including organising appointments, providing patient education and ensuring that results (once returned) are integrated into the patient record.

Undertake Urinalysis and work with other allied health professionals to ensure that results are actioned in a timely manner. Undertake a range of other tests within the scope of your training and competence. Work closely with the wider team and the practice procurement lead to ensure that a range of medical supplies are ordered and stocked in a timely manner. Administration of influenza vaccine, Shingrix, pneumovax and vitamin B12 injections under PSD once trained and in accordance with the stipulated schedule.

Carry out wound care after an initial assessment by the practice nurse. Undertake doppler studies with an automated machine (once training has been provided) Remove wound closure materials including sutures, clips and staples. Assisting coil fits and minor surgery; preparation of equipment /completion of records Ensure all clinical rooms are adequately stocked and prepared for each session. Provide support during minor operations as required.

Support the practice nurse with health promotion programmes and deliver opportunistic health promotion where appropriate. Ensure fridges are cleaned routinely in accordance with extant guidance. Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy. Education and Training To develop the scope of professional practice in order to develop this role to meet the service requirements in accordance with professional guidelines.

Completion of Care Certificate or NVQ in care; this knowledge needs to be regularly updated. Completion of mandatory training (face to face or online) as stipulated by practice policies. Undertake annual training for influenza vaccination administration. Support the development of the nursing team and the wider practice through participation in regular audits.
