We are looking for an enthusiastic **Prison In-Reach Worker** and become a catalyst for positive change within the prison system and local communities. In this role, you'll be instrumental in assessing and engaging with individuals to ensure they receive the right intervention at the right time, tailored to their unique needs. You'll work with people transitioning from custody settings, providing clinical interventions, support plans, and court reports as needed.

**Hours**: 37.5 per week

**Salary**: £27,449.53 - £31,529.41 pro rata (dependent on experience)

**Allowance**: £4,072.06 Inner London Weighting, Pro Rata

**Responsibilities**:
In this role, you'll be instrumental in engaging with complex people who are caught in a cycle of re-offending and substance misuse, addressing their immediate needs and working closely with partners both within the prison and in the wider community. Your aim will be to improve Continuity of Care and to promote social integration through trauma-informed practices and empower individuals to change their lives, grow, and embrace life's full potential.

**Role-specific responsibilities**:

- ** Comprehensive Assessments**: Act as the main coordinator providing comprehensive assessments and coordinate resettlement plans from prison by liaising with relevant agencies.
- ** Advocacy**: Serve as the navigator for interventions and advocate on behalf of the service user.
- ** Action Plans**: Carry out comprehensive service user assessments, develop individual action plans, and identify community resources to enable progression in addressing offending/substance use and support the individual to reintegrate back into their community.
- ** Risk Assessment**: Work closely with prison security departments to assess risk and develop strong links with partner agencies within the prison system.
- ** Team Collaboration**: Work as part of a team to identify individuals with a history of substance misuse and re-offending resulting in custodial sentences.
- ** Information Sharing**: Support information sharing and shared processes between all relevant partners to ensure a safe, seamless, and successful resettlement plan from prison.
- ** Community Support**: Collaborate with local community services and/or prison establishment to ensure the provision of Naloxone and overdose prevention information at the point of release.
