**Pay Rates**:
- £23,000pa
- Out of hours shift payments - £20 M-F & £160 weekends
- 45p tax free mileage rate
**Shifts**:
- Full time Monday to Friday - 9am - 5pm
- There will be paid on call duties on a rolling rota basis. **What's in it for You**:

- Clean and valid driver's license with access to a reliable vehicle.
- A Level 3 NVQ qualification in adult and social care or equivalent is desirable.
- Proficiency in written and spoken English with strong communication skills.
- Experience in care plan management. **Key Responsibilities**:
- Work closely with our Reablement Team Manager, Coordinator, and other professionals involved in the care and support of our service users to develop care plans.
- Co-design care and support plans with customers and their families, ensuring their personal care needs are met, dignity is maintained, and individual outcomes and preferences are reflected.
- Conduct relevant risk assessments to ensure the highest quality of care.
- Promoting and facilitating client independence.
