**Salary**: £23,000 per annum

- On Call Allowance: £150 per weekend (One weekend oncall included in the salary, 2nd-weekend oncall £150 p/w )

**Reporting To**: Lead Coordinator/Registered Manager

**Job Purpose**: Techcited Ltd is working closely with a client who is looking for a Care Coordinator. The Care Coordinator is essential in delivering high-quality care by efficiently organising the deployment of Care Assistants, enabling our clients to remain safe and comfortable in their own homes. This role requires an exceptional combination of organisational skills, effective communication, and a supportive attitude.

**Key Responsibilities**:

- **Rostering of Care Assistants and Support Plan Preparation**: Ensure Care Assistants are scheduled and allocated efficiently, guaranteeing care delivery is timely, safe, and in accordance with individual care and support plans as well as the availability of Care Assistants.
- **Office Administration and Operational Support**: Collaborate closely with the Lead Coordinator/Registered Manager to facilitate smooth care operations, arranging cover for any Care Assistant absences due to illness, holidays, or emergencies.
- **Referral Management**: Quickly process new client referrals and adjustments to care and support needs, ensuring responsiveness to changes.
- **Optimisation of Scheduling**: Adjust Care Assistant schedules to effectively balance client needs with resource efficiency, considering skills, experience, and logístical considerations.
- **Communication and Coordination**: Maintain strong communication with clients, their representatives, and healthcare professionals to ensure the delivery of high-quality care. Manage emergency on-call duties and address crises as necessary.
- **Record-Keeping**: Keep comprehensive electronic and manual records, including incident reports, staff rotas, and mileage, ensuring confidentiality and data security.
- **Administrative Responsibilities**: Good administrative and IT skills. Perform administrative tasks, compile reports, and promote continuous service improvement in accordance with company policies and procedures.

- Good local geographical knowledge to ensure efficient scheduling. (Roads / routes / traffic levels).
- Ability to remain calm and make clear decisions under pressure, utilising initiative in emergencies.

**Additional Requirements**:

- Flexibility in working hours and a commitment to continuous professional development.
- A satisfactory enhanced DBS check.
- A valid driving license and access to a vehicle
