The Gedling Living Well Team are looking for an enthusiastic experienced Community Care Officer with experience of working with adults with mental health issues and/ or physical disabilities. This is an exciting opportunity to be part of a multi-specialist team offering assessment and commissioning support to people in the community.
The CCO role in mental health focuses on reablement, helping people by providing direct support to increase their independence and reduce the need for ongoing support. Reablement involves identifying personal outcomes, for example support with housing issues, activities of daily living, building confidence, travel, accessing the community and getting involved in social groups and or volunteering.
In addition to reablement you will also work with people who have either mental health or physical disabilities, to undertake community care assessments and reviews to assess eligibility under the care act for care packages. Work will also include completing assessments and commissioning care to facilitate discharges from mental health hospitals.
You will work in a strength-based way and aim to make a real difference to the lives of the individuals that we work with and their carers.
Good communication, time management and IT skills are essential.
You will need to have experience in positive risk taking and risk management.
This is one of the most rewarding areas of social care and the team offer good support within a friendly atmosphere. You will receive regular supervision and have access to training.
