REHABILITATION ASSISTANT PRACTITIONER PATHWAY 1 WRAP AROUND TEAM
37.5 hrs per week

An exciting opportunity has arisen for Rehabilitation Assistant Practitioners to join the West Suffolk community healthcare teams, concentrating on follow up home visits for those being discharged home from hospital under Pathway 1.
We are looking for passionate and caring people with skills and experience of working with adults who have varying medical conditions or complex conditions. The post holder will work autonomously within peoples home environments in the community, holding a caseload whilst working under the guidance of the occupational therapists. The postholders will have an awareness and understanding of the benefits of reablement to improve peoples daily lives, delivering therapy programmes and feeding back to the occupational therapists. The ability to establish rapport with person receiving therapy and working closely with health and social care colleagues, carers, family members and other professionals is important.
