Suffolk Stroke Early Supported Discharge Team
0.64wte permanent 24hours per week
Due to service expansion and re-design, an exciting opportunity has arisen within the Suffolk Stroke Early Supported Discharge Team for Band 3 Rehab Assistant roles.
The Suffolk Stroke Early Supported Discharge Team provides specialist individualised stroke rehabilitation, in order to support stroke patients and their carer’s in the first 6 weeks of discharge from hospital to home.
We are looking for a passionate Band 3 Rehab Assistants for part-time, permanent positions. We work as an interdisciplinary team which operates 5 days a week, Monday - Friday, excluding Bank Holidays; however due to changes in service provision; the post holder might be required to deliver 7-day service or engage in a wider service delivery model in the future. The interdisciplinary team consists of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Neuropsychologist, Assistant Practitioners and Rehabilitation Assistants.

**Key Tasks & Responsibilities**:
To work under the direction of qualified staff in the performance of specifically documented tasks and treatment plans for patients following a stroke.
To provide personal care to patients in their own home, with a philosophy of reablement.
With support and guidance, carry out planned programmes of care and rehabilitation to meet individual patient and families’ circumstances, including due regard for specific clinical conditions related to communication, cognitive, emotional and physical impairments resulting from a stroke.
To ensure that patients, families and carers are involved in the planning and implementation of their stroke rehabilitation care plans.
To work collaboratively with other services that may be involved in the care of the patients for instance, social services and the volunteer sector.
To attend and contribute to relevant multidisciplinary meetings and client reviews as appropriate.
Update patients records in a concise, accurate and timely manner in line with service standards
Communicate to Nurses and Therapists regarding changes in a patient’s condition, their progress in rehabilitation, and barriers to the patient achieving their rehabilitation goals.
To be aware at all times of the needs of patients during their rehabilitation activities, promoting/encouraging independence and restoration of function.
Provide simple equipment and advice in its use to patients and their relatives with direction from senior colleagues. This may involve the transportation of some items of equipment.
To use equipment to assist in the transfer of patients and in assisting patients to reach their rehabilitation goals.
