**We’re Looking for a Complex Care Coordinator**:

**What**
As a complex care coordinator within Superior Healthcare’s complex care division, based in an office environment, you will have your own client caseload and will work with clients, their families, carers and commissioners. You will work with a team to support clients, by helping them maintain their health and enhancing their independence.
**Hours of work**:
37.5 hours per week, (plus additional hours according to the needs of the business and participation in telephone on-call back up).
**Day-to-day duties will include**:
- Coordinating care workers and nurses work patterns to ensure they provide support
- Ensuring clients receive high-quality care both safely and compassionately
- Organising, coordinating and managing packages of care
- Supervising the deployment of care services in accordance with agreed standards
- Ensuring staff have the necessary, up-to-date training and support to excel in their roles
- Working with team members to establish effective employer-employee relationships
- Encouraging team members to maintain excellent levels of care and support
- Acting as an ambassador for Superior Healthcare and upholding the company’s core values
**Role Requirements**:
- Experience from a previous care coordination role
- Customer Service experience
- A QCF Diploma Level 3 or other relevant vocational qualification
- A genuine interest in working within a caring, collaborative environment
- Excellent interpersonal skills and a ‘team player’ attitude
- Good IT skills, including Microsoft Office packages (mainly Word, Excel, Outlook and Teams).
- Excellent organisational and administration skills
- High attention to detail
- Excellent communication skills
- Ability to work efficiently under pressure and to tight deadlines
- Ability to work independently and as part of a team
- Satisfactory enhanced DBS disclosure
- Full, clean UK driving licence
**Our offer**:
We are looking to increase the numbers of coordinators in our team. The successful person will be offered a salary of between £22,500 and £25,000 depending on experience.
