GENERAL RESPONSIBILITIES
- To provide a high standard of patient focused care
- To work as part of the practice and primary care network team
- To work within the guidelines and policies of the practice & Primary care Network
- To be familiar with practice policies and protocols
- To participate in educational events as advised by the practice
- To attend various practice meetings as requested

PRINCIPAL DUTIES AND RESPONSIBILITIES
- Responsible for patient call and recall and they may also assist the patients in preparing for their follow up clinical conversations they have with their primary care professional
- Respond to all queries and requests for assistance from patients
- Run searches using the relevant clinical systems and the Ardens system
- Maintain the clinical computer system in an accurate and secure manner
- Ensure data security at all times and report faults to Operations Manager
- Signpost patients and their carers to appropriate support both inside and outside the practice
- Responsible for document management within the practice; electronic and paper documents
- Ensure appropriate referrals to Community Link Workers for patients and carers
- Responsible for organising regular MDT, inviting the appropriate attendees and pulling together patient lists and agendas, attending meetings where appropriate, taking minutes and typing them up.
- Take an active role in supporting patients carers and relatives, signposting them to appropriate clinicians and services.

Case management of identified groups of patients including recall for Year of Care, Heart failure, Spirometry, flu and Covid-19 vaccine booster program
- Proactively identify patients who require a review or need to be recalled, contact patients, send letters and ensure the YOC planning paperwork is completed.
- Identify which clinician a patient may need to see depending on their health conditions and specific needs.
- Ensure the appropriate appointments have been made in accordance with the patients annual review
- Help people to manage their needs, answering their queries, referring and signposting to Community Link Workers or other services if needed.
- To proactively work with patients who have been referred on a cancer pathway, to track their referral and ensure safety netting is in place for these patients
- To case manage other clinical registers, as necessary and in discussion with the practice team
- To work with the PCN and practice team on measures to improve quality of care, including running searches and other relevant work to underpin audit and improvement of services
- Lead on the organisation of the annual flu programme, calling patients and booking appointments. Pro-actively contact reluctant participants and encourage attendance.
- Take a lead role in the practice for call / recall of patients who are due covid-19 booster vaccine. The post holder may need to travel to a covid-19 vaccination sites in order to assist with the delivery of the program.
- Responsible for the Cervical Cytology call and recall system

Clinical System / Medical Records
- Ensure total familiarity with the EMIS clinical system
- Book appointments accurately for call / recall ensuring sufficient information is recorded
- Extraction of necessary data from patient clinical record in order to provide information for regular MDT meetings
- Ensure electronic document correspondence is dealt with and distributed in a timely manner as per practice protocol
- Ensure postal correspondence is dealt with and distributed in a timely manner as per practice protocol.
- Booking and cancelling appointments where necessary

Miscellaneous
- Provide cover for members of the administration and reception team during periods of sickness and annual leave, this will involve travelling to other sites.
- Directing patient complaints to the relevant member of the management team where appropriate
- Work effectively with individuals in other agencies to meet patients’ needs
- Effectively manage own time, workload and resources
- The care co-ordinator will be responsible for organising regular practice meetings, inviting the appropriate attendees whether that be face to face or via MS teams and pulling together patient lists and agendas, attending meetings where appropriate, taking minutes and typing them up
- Receive and make calls as required answering any queries that arise
- Divert calls and take messages as appropriate ensuring accuracy

Training
- Participate in any in-house and out of house training in order to support and enhance staff skills
- Ensure maximum use of resources to gain the necessary knowledge, skills and attitudes to provide the service required
