**Job summary**:
We are the Motor Neurone Disease Care Centre (MND), Birmingham - one of the largest specialist centres caring for people with MND in the UK. We are passionate about supporting people living with MND from diagnosis to end of life.

We are excited to be recruiting a 12 month fixed term Band 6 nurse who shares our passion to strive towards excellent care in MND, who is keen to develop the service and improve all aspects of patient care. We are enthused to recruit someone who is ready to work across acute and community services, as part of a small specialist team which delivers care and works collaboratively with 10 community multidisciplinary teams. A car driver with their own car is essential.

This is a fantastic opportunity to join the wider MND community and work alongside the MND Association to manage symptoms, support future planning for End of Life care and maintain quality of life throughout the disease progression.

The post holder will be based at the Queen Elizabeth Hospital but will work closely with different NHS trusts across the West Midlands and must be willing to travel to different locations in the West Midlands. This post is full-time and investing in the health and wellbeing of our staff, including a commitment of offering flexible working. This role will not include working bank holidays; weekends or night shifts. We will offer our staff a wide variety of training and development opportunities, to support their personal and career development objectives.

**Main duties, tasks & skills required**:

- To act autonomously within a supportive team, managing a caseload of MND patients, delivering individualised and personalised direct patient care.
- Provide a specialist nursing service and advice in a variety of clinical settings and disciplines for staff, patients, their families and carers whilst effectively liaising with multidisciplinary colleagues.
- Participate in nurse led, multidisciplinary and consultant led clinics as well as carry out visits within a patient's home.
- Participate in multidisciplinary team meetings and coordinate management plans.

**Qualifications**:
**Essential**:

- Registered Nurse with valid NMC registration
- Evidence of post registration CPD / post registration qualification in appropriate area of speciality or professional knowledge supplemented by specialist experience and short courses to FHEQ Level 6

**Experience**:
**Essential**:

- Evidence and ability to revalidate as required by the NMC
- Significant post registration experience, with evidence of passion/ enthusiasm and knowledge for the associated area of specialist practice
- Evidence of achievements and career development in current post and of on-going professional development/ competence
- Demonstrate specialist knowledge underpinned by theory and CPD relevant to speciality
- Experience and enthusiasm for teaching/assessing and supervising others in a clinical setting
- Experience of working in a multi-disciplinary and cross agency work environment
- Demonstrate understanding / experience / willingness to be a practice-based supervisor / assessor as detailed in the NMC
