You will undertake the role of INT Senior Administrator working alongside health, social care and voluntary sector partners to provide a dynamic integrated model of care for local patients.

Your role will be to ensure effective and accurate maintenance of systems, support service planning and delivery, monitor performance and support with data retrieval. You will manipulate data to assist in data analysis and interpretation.

Using your excellent communication and interpersonal skills, you will develop excellent working relationships with a range of health, care and community partners and expertly facilitate regular integrated team meetings to ensure local people and patients achieve positive outcomes. You will be first point of contact for all service users and local people.

The post holder will be a key member of the Integrated Neighbourhood Team and will be supporting the wider including the Integrated Neighbourhood team Co-ordinators, Associate Psychological practitioners, vulnerable persons practitioner/ homeless healthcare practitioner and the wider multi-professional team to ensure everyone is working together to provide more local and personalised services for local people within the community.

The post holder will be the link between primary care, voluntary sector, social care and community health provider services.

The INT Senior Administrator role provides assistance and support with front line duties as part of the multi-disciplinary decision making processes, assessment and intervention, assisting in the provision of a quality support service; liaising with patients, families (and carers), members of the Primary Care Networks and other agencies; whilst ensuring that patients and service users access the right professional/service required.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
