JOB TITLE: Supported Housing and Recovery Worker
RESPONSIBLE TO: Team Leader
JOB PURPOSE: Responsible for supporting and inspiring tenants, who are recovering from addiction issues to move forwards in their lives. Recovery is as individual as the service user we support and we are looking for a motivated and caring individual who can use their own initiative to establish support plans, support their implementation and support our tenants to work towards becoming an asset to the local community.
KEY DUTIES AND RESPONSIBILITIES: Please note this is a general description for a Housing and Recovery worker role, but duties are personalised to the Tenants individual needs.
1. To develop and sustain warm and trusting relationships with the people we support, promoting their self-esteem, happiness, and emotional health.
2. To complete daily welfare checks to all tenants and conduct weekly health and safety inspections.
3. Encourage and support people in expressing their needs, views and concerns and enable them to make choices and decisions and to participate as fully as possible in planning and decision-making processes.
4. To support people in developing a socially valued lifestyle which includes a varied range of culturally and age-appropriate experiences, building on the person’s strengths, interests, and aspirations. This may include walking, hiking and other community based activities that promote well-being and the adoption of a healthy lifestyle.
5. Facilitating the attendance of 12 step fellowships and community based social activities in line with the individual interests and needs of the person and in line with individualised support plans.
6. To role model to the people we support in general housekeeping, such as cooking, domestic tasks, taking pride in their own personal space, preparing meals, laundry and shopping.
7. To create opportunities for the people you support to develop and maintain friendships and personal relationships, encouraging maximum participation and inclusion within their communities.
8. To achieve a range of appropriate communication techniques in accordance with the individual needs of the person.
9. To provide support to people in their contacts with healthcare specialists, social services, housing departments and other external agencies, e.g., attending relevant appointments, helping to explain information, and completing relevant paperwork in partnership with individuals.
10. To ensure that people receive all necessary advice, care, and regular health checks to maintain their physical health and well-being. To promote nutrition, relaxation, exercise, and a healthy lifestyle.
11. To manage site petty cash and the appropriate documentation.
12. Empower individual and encourage and support people in undertaking training and employment opportunities to give back to local community.
13. Assist or supervise people as necessary with their medication and conduct weekly medication audits.
14. Work within the ethos and values of Bright Start Homes, to develop and operate best practice and to constantly promote and support people’s independent living and participation in their community.
15. To help as part of the care and support team to develop and update individual support plans, which cover daily routines, health action plans, likes and dislikes and key contacts etc.
16. To ensure daily notes are completed to a high standard and that notes are documented in a clear and professional manner, avoiding breaches of confidentiality and any use of slang terms or demeaning terminology; and to ensure that all communications are consulted at the beginning of each shift.
17. To contribute to the support of individuals with their annual person-centred reviews, liaising with relevant personnel (e.g., family, friends, and social worker) and ensuring, in conjunction with the relevant team leader, that all preparatory paperwork is completed, and any actions and minutes taken are agreed and implemented.
18. Where requested to act as lead support for individuals and advocate for them for example at PIP assessments.
19. To support people who may express their frustrations and needs through challenging behaviours by using appropriate strategies, management guidelines and intervention frameworks as specified by their Person-Centred Plan and support agreements.
20. To be responsive to the individual needs of people we support within the framework of their Person-Centred Plans and to respond flexibly to changing needs.
21. To assist people with the understanding and operation of their own benefit and personal banking arrangements in accordance with policy and procedure and to support people in claiming their full benefit entitlement.
