**About Us**

At Beacon Case Management, we provide Case Management services for clients who have suffered complex and catastrophic injuries. Our bespoke service is client-focused, supporting clients to recruit, support and offer training to their own support teams.

**Job Role**: Young Person's Support worker

This is an exciting opportunity to support a 15 year old male with cerebral palsy living in the community, in his own home. The successful applicant will work within a small team at the clients home, working closely with the family.

The client enjoys going out in his electric wheelchair, eating out, watching films, listening to music, gaming on the Xbox, playing pool, swimming and playing in water, football and watching YouTube.
- Guide and facilitate our client’s journey through rehabilitation
- Collaborate with the wider team and take part in regular review meetings
- Provide stability and dedication to the role
- Focus on improving the client’s quality of life

This role consists of the following shift pattern Monday to Friday: 7.30am-9.30am and 3pm-9.30pm. Weekends are as needed and will include longer shifts (no split shifts on a weekend). The majority of the contracted hours will be worked during the week, so it's important to have availability during the hours above before applying.

**Responsibilities**

This role involves support that is personalised for this individual in the form of a support plan. Support duties will therefore include assisting the client in their daily life, and includes a variety of duties.

Example of duties in the role:

- Supporting the client with personal care needs
- Supporting the client in their own home and local community
- Supporting the client before and after school
- Sometimes supporting the client on holidays in the UK and abroad
- Supporting the client with therapy activities and rehabilitation programme
- Supporting the client with meal preparation and shopping
- Completing daily records
- Liaising with the case manager
- Supporting client to manage their budget
- Supporting the client to manage medication

**Person Specification**

**Essential requirements of the applicant**:

- Can demonstrate a willingness and ability to learn
- Understanding of safeguarding
- Full driving licence
- Compassionate and caring
- Skills in working autonomously when required however knows when to seek advice and assistance from the wider team
- Excellent communication skills

**Desirable requirements of the applicant**:

- Evidence of qualifications in Health and Social Care equivalent to the care certificate/ NVQ level 2.
- Previous experience of working with people who have physical disabilities and communication aids.
- Previous experience of working within a multi-disciplinary team including therapy and rehabilitation
- Understanding of regulations and legislation within health and social care
- Non-smoker
- Have skills in building trusting rapports with clients

**Contracted Hours**: Up to 28 hours per week

**Rota arrangements**: given 8 weeks in advance

**Contract arrangement**: Permanent

**Pay details**:
£12.50 weekday day

£NMW weekday nights (rare)

£13.50 weekend day

£NMW weekend nights (rare)

£13.50 bank holidays

**Holiday Entitlement**:5.6 weeks per annual (pro rata)
