Critical Care Outreach Nursing Lead

To operationally lead the Critical Care Outreach and to provide advanced clinical expertise, knowledge and skills utilising clinical judgement skills and decision making in the care and management of acutely unwell patients. To lead and participate in all variety of clinical emergencies, such as NEWS calls, medical emergencies, and cardiac arrest calls, as well as assisting in the provision of level 2/3 care outside of the Critical Care Unit. To lead and innovate nursing, allied health care professional & medical practice in the specialty of Critical Care Outreach To ensure all Trust staff are best prepared to manage life threatening emergencies encountered during their work To be a highly visible and proactive leader acting as a clinical resource and ensuring the sustained delivery of high standards of nursing care and service delivery To take responsibility for own evidence-based practice and to implement and add to the body of knowledge that supports clinical practice related to acutely unwell patients within the Trust, nationally and internationally. To lead on and promote educational strategies for professional groups regarding the assessment, care, interventions and management of the deteriorating patient, patients who are acutely unwell, and patients who have been recently discharged from the Critical Care Unit and to assist the Critical Care Team with patients who have critical care rehabilitation needs.
To lead on and assist in the facilitation, organization and provision of education and training on the deteriorating patient. To have a role with the Trust Quality Improvement team, leading QI initiatives within Critical Care and the wider remit of Outreach service To be responsible for exploring current practice, identifying areas for development, and planning the appropriate change using current evidence to evaluate outcomes To be responsible for the quality of the patients environment and lead on facilitating changes which improve the patients experience To play a key operational role and maintain a proactive approach to managing patient flow through the service. The post holder will deliver an 30% clinical component to their role and 70% related to leadership and management, education, and research. To participate in the on-call duty senior nurse rota To maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise.
To play a key operational role in monitoring patient outcome within the service, ensuring measures flagged from benchmarking data are embedded in clinical practice and managed proactively on a daily and strategic basis.
