Salary: £32,000 - £38,000 per annum, depending on experience

Job Description:

This challenging yet rewarding role requires a highly motivated individual to provide intensive outreach support to customers in independent accommodation.

You will work closely with customers, colleagues, and Senior Managers to ensure the effective operation of our Support Service, promoting a cooperative and supportive environment.

Key Responsibilities:

  • Provide intensive outreach support to customers with varying needs, including mental health challenges and other health-related issues.
  • Work closely with customers to develop new services that meet their needs and promote a sense of community.
  • Collaborate with colleagues to ensure seamless service delivery and high-quality outcomes.

Required Skills and Qualifications:

  • Demonstrable experience supporting customers within an Intensive Housing Management service.
  • Experience supporting customers with mental health needs, other health-related issues, and training/employment.
  • Possess knowledge or understanding of housing support needs and benefits.
