Dementia Support Worker

Dementia Support Hampshire & IOW is a small independent charity that provides emotional support, information, and signposting for people living with dementia and their families in Hampshire and the Isle of Wight.

Thanks to a new partnership with SGN we are expanding our services during 2024 and are looking to appoint a Dementia Support Worker to visit people living with dementia in their home across the Isle of Wight, to provide information about dementia, care and support pathways, community support, benefits and energy advice.

The main duties of the role will be to manage your own caseload to visit people in their home or to meet with them at one of our Support & Social Groups in the community, to provide information and help them to to access services and benefits based on their own personal needs. You will also arrange information days in the community and attend community events to raise awareness of the service.

**Responsibilities**:

- Effectively plan to ensure that you are managing your own caseload.
- Educate and inform individuals about available resources and services, provide fuel debt advice and assist with sign up to the Priority Services Register and other energy safeguarding support schemes
- Collaborate with internal and external professionals to coordinate access to services to ensure the best possible outcomes for individuals
- Advocate for individuals and help them navigate the health & social care system
- Update our CRM (database) to ensure all actions and outcomes are recorded

**Experience**:

- Good problem solving and decision-making skills and the ability to prioritise and plan your work
- Lived experience of dementia through caring for a loved one would be an advantage, or experience of working with/caring for people living with dementia or vulnerable adults.
- Experience in filling in forms to claim benefits/blue badges would be advantageous.
- Strong communication skills, both written and verbal, to effectively interact with individuals and their families, community health professionals, community partners, staff and volunteers
- Understand the importance of data collection and the need to evidence impact, as well as experience of using a CRM (database)
- Ability to work independently as well as part of a team

This position requires a compassionate individual who is dedicated to making a positive impact on the lives of others. The Dementia Support Worker role will play a vital role in helping individuals access the support they need to thrive.

Use of own car is essential. Mileage expenses will be covered.
