Role: Care Coordinator

**Salary**: Negotiable depending on your experience
Hours: Full time (9am to 5pm) with additional on call on a rota basis and may require going out to deliver care out on the field from time to time.

Role Overview:

- Maintaining a computer database and knowledge of People Planner or similar systems and keeping information up to date at all times.
- Work closely with the Registered Manager to ensure compliance with all office systems and procedures.
- Complete Risk Assessment and supporting with Care Plans.
- Answering incoming telephone enquires.
- Ensuring all client visits are covered and allocating Care Assistants to the correct shifts.
- Process new clients referrals.
- Organise and coordinate weekly rotas and care services.
- Work with recruitment to ensure sufficient staffing levels.
- Carry out on-call coordinating and response duties outside normal office hours as agreed.
- Dealing with any queries and supporting clients and family in a calm and professional manner.
- Working with professionals and organisations to ensure person centred packages of care is being delivered
- Ensure that rotas are well planned and organised and take into account for routes and travel time

**Job Types**: Full-time, Permanent

**Salary**: £21,000.00-£28,000.00 per year
