We are seeking a skilled Care Coordinator to join our team at Routes Healthcare. As a Care Coordinator, you will play a vital role in scheduling and coordinating care visits to ensure our clients receive exceptional care.

Key Responsibilities:
  • Coordinate the delivery of care using our care scheduling system
  • Adapt to changes in schedules, including staff availability and new clients into existing care runs
  • Ensure all staff attend staff meetings, supervisions, and training within required timescales
  • Assist the Registered Manager with general office administration tasks
  • Manage on-call duties on a rotating basis
