We are seeking a highly skilled and compassionate Care Coordinator to join our team at Routes Healthcare in Rochdale. As a Care Coordinator, you will play a vital role in scheduling and coordinating care visits to ensure our clients receive exceptional care and support.
Key Responsibilities- Coordinate the delivery of care using our care scheduling system
- Adapt to changes in schedules, including staff availability and new clients into existing care runs
- Ensure all staff attend staff meetings, supervisions, and training within required timescales
- Assist the Registered Manager with general office administration tasks
- Manage on-call duties on a rotating basis
- 25 days paid holiday, 8 days paid Bank Holiday, and an extra day off for your birthday
