**Care Co-ordination Job Description**

The main duties of the role of Care Co-ordinator is to manage rostering effectively so clients receive the care at the commissioned call times, and that carers have sufficient travel time and breaks built into their daily runs. However there are a number of other duties as listed below:
Reporting to the Branch Manager, the Care Coordinator is responsible for the effective management and supervision of Care workers and supporting the Branch Manager to ensure the branch is providing an excellent provision of service to our customers, clients and colleagues on a daily basis. Ensuring compliance with national minimum standards and using effective rostering to fully utilise staff hours and drive employee engagement.

Allocation of care workers to customer visits by using effective rostering, ensuring continuity of care and taking into account locations, skills required and customer preferences.
Take responsibility for new referrals from private customers and social services teams, ensuring full information is received and appropriate care planning is undertaken. Assist Branch Manager with identification of further business opportunities outside of the current client base.
Matching care workers to customers, ensuring care workers are notified of all conditions relating to the assignment, including care worker’s rate of pay.
You must have good IT, communication and office skills
Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care
Dealing with queries from clients and families in a calm and professional manner.
Ensuring weekly KPI’s are completed
Organising and supporting with Staff Supervisions and Appraisals
Attending and participating in Staff team meetings
Completing log book drop offs
Deputy care co/field care have their own clients. It will be your job to ensure your clients are happy and all paperwork is present/up to date

Carrying out visits to customers’ homes to carry out initial Customer Assessments and continuous monitoring
Risk Assessments, administration and review of new support packages
- Customer Reviews
- Support Worker supervision & support

Any other tasks required by the Deputy Manager which she/he deems necessary for the needs of the business.
Report all expenses in an accurate and timely manner to Lotus finance team to ensure all payments are reimbursed by 20th of each month.

Skills, Experience and Qualification:
Excellent organisational, administration and dairy management skills
Excellent communication skills
Strong IT skills including Excel, Word and good typing speed
Experience of a scheduling or rota system
Ability to work to tight deadlines, paying attention to detail and within a fast paced office environment
Proven background in delivering high quality customer care/services
Ability to work independently
Ideally previous carer/care industry experience
Ideally qualified to NVQ level 2 or higher in a relevant health and social care discipline
You must have good IT, communication and office skills

You must demonstrate commitment to working within a team, have a flexible approach to working hours and experience within the Care industry. This role will involve an element of on-call duties and out of hours emergency care, therefore a driver with access to your own vehicle is essential.

Please note the above is not an exhaustive list and can change from time to time depending on the needs of the service. Therefore there is an expectation that you will be able to be flexible to meet any changes to business needs. You are expected, as a minumum, to work alternate weekends on-call for your branch, or to support neighbouring branches if needed, under the instruction of your line manager.
