An exciting and challenging opportunity has arisen for a committed Assistant Practitioner to join our innovative Specialist Autism Team within Joined up care Derbyshire.

This is a newly commissioned service to strengthen the pathways for adults with autism.

We are looking for individuals with an understanding of the support needs of people with Autism who are inpatients or requiring additional support and at risk of a hospital admission.

The focus will be to offer support within the community with access to a range of services available.

Applicants should be confident leading a specialist multidisciplinary team and forming strategic relationships with a range of health, social care and commissioning partners across Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

Reporting to the Operational manager within the learning disability and autism service you will be responsible for leading and delivery for both the operational and strategic development of the team.

You will also be responsible for ensuring the team meet the outcomes they are commissioned for and ensuring excellent person cantered care for individuals with Autism.
