**Purpose**:To proactively identify Carers and provide them with information, guidance and support including supporting them to access services, peer support and short break opportunities in order to reduce the impact of their caring role.

**Specific Responsibilities**
- Undertake community engagement activity to identify and support hidden carers
- Introduce new Carers to the service, meeting them at their home or in the local community
- Undertake person centred asset-based assessments with Carers to help understand the impact of their caring role and develop support plans that help to reduce this impact
- Undertake regular reviews of Carers needs/support plans
- Upload Carers information to the on-line central management system (CMS)
- Inform the Carers of the support available to them via this service and other services to meet their needs.
- Make outgoing referrals to facilitate Carers’ access to other local services/organisations where appropriate.
- Support Carers to self-help i.e. utilising national and online resources, connecting with family and friends
- Inform Carers of their rights under relevant legislation and policy to support them to make informed decisions
- Support Carers to access and navigate appropriate community based and statutory services for the cared for person thereby supporting the carer in their caring role and allowing the carer respite
- Refer Carers with appropriate needs to Adult Care Services for a statutory carers Assessment
- Encourage and support Carers to plan for emergencies i.e. by recording the care and support needs of their cared for and sharing this with those that can step in should an emergency occur.
- Support Carers to understand the condition of the cared for person to enable them to provide appropriate care including signposting to appropriate service i.e. dementia services, mental health services
- Facilitate community drop in’s
- Support the development/facilitate support groups, peer support, training sessions and short break opportunities for carers i.e. coffee and chats, activities, trips, and social events
- Attendance at community events
- Contribute to the newsletter for local carers and professionals
- Deliver carers awareness training/briefing sessions to community members and professionals
- Provide accurate monitoring reports as requested to support service reporting
- Achieve individual performance management indicators as set by the Service Manager
- Responsibility for promoting and safeguarding the welfare of children and adults
- Provide support throughout the wider carers service as required
- Support the induction, training and supervision of social work students and volunteers

**Personal support**
- Attendance at bi-monthly staff meetings
- Develop and attend a regular informal peer support mechanism with other support workers throughout the organisation
- Attendance of quarterly My Journey meetings
- Work within organisational policy and act in the best interests of the organisation at all times
- Work within the organisations Safeguarding Policy, including completing safeguarding training to ensure a good understanding of safeguarding concerns, including potential abuse/neglect of adults/children and young people
- Work within the organisations Quality, Information Governance and Caldicott Principle Policy. This will include following processes in line with the ISO Quality Management System and the Information Governance Tool Kit

- Experience of using on-line recording systems
- Experience of producing written reports to high and detailed standards
- Experience of using Microsoft Office e.g. word, excel, outlook
- Experience of working in the voluntary sector
- Experience of supervising students/volunteers
- Experience of supporting carers
- Knowledge and understanding of carers rights
- Knowledge and understanding of carers assessment process

**Education /Qualifications**

e.g. academic or professional qualifications
- Qualification / training related to the support of adults and/or young people
- Safeguarding adults L1 & L2
- Safeguarding children L1

**Skills/Abilities**

e.g. analytical skills, computer skills
- Ability to listen and communicate effectively with a wide range of individuals
