**Role Purpose**

You will work closely with a group of people who have health conditions each year, supporting them to profile their skills/skills they wish to develop, and jointly create and achieve an action plan that will enable them to achieve their work goals and becomeindependent, active members of their community.

To carry out this role, the post holder will have significant working knowledge of the Dundee City area and key employability stakeholders and sectors. The post holder will be responsible for developing positive relationships with those they are supporting,but also fellow partners on the Pathway, who they will work with closely, and wider stakeholders, such as local third sector organisations and businesses to ensure people a broad offering of experiences and opportunities.

**Key Duties & Responsibilities**
- To meet ambitious targets in supporting people with health conditions, achieving positive outcomes for all
- Working 1:1 with people, identify barriers they face and provide support and strategies to overcome them
- To provide 1:1 employability support that is tailored to the person’s personal circumstances
- To engage directly with people to understand their wants, needs and aspirations and determine the best opportunity for each to volunteer, and provide the support they need in advance of volunteering
- To manage all elements of the person’s experience from point of referral to completion of their volunteering opportunity, and moving on to education, training or a job
- To identify, develop and maintain excellent relationships with charity and not-for-profit partners across Dundee, as well as referral partners and agencies
- Ensure diverse, high quality volunteer roles are available that reflect the needs and interests of people
- To manage the risk assessment, quality assurance, compliance, and audit processes as appropriate
- Ensure monitoring, impact and evaluation information and data is collected and that the project meets it’s agreed targets, reporting format and schedule. Assist the Delivery Leader by preparing reports to funders

**Person Specification**

**Skills**
- A sound and comprehensive understanding of the particular challenges and barriers people with health conditions face to entering the work place
- Understanding of how to assess the needs of an individual using a person-centered approach
- Excellent organisational and IT skills
- Excellent people skills with the ability to build professional, long-term relationships with others
- People-focused, and adaptable - able to go from strategic conversations with key partners to chatting with a nervous potential client
- Ability to manage a busy workload and prioritise accordingly

**Experience**
- Significant experience working and supporting people directly, particularly those who face challenges and barriers such as health issues
- Demonstratable relationship management experience with a wide range of stakeholders
- Experience in using a CRM (customer relationship management) system

**Other**

Having a regular presence in the area, to work with partners and the people engaging the service is essential for this role (in line with COVID restrictions). When home based, internet access to enable remote working is essential. IT equipment and infrastructurewill be supplied.

A driving license and access to a vehicle would be beneficial to carry out this role.

This role requires membership of the PVG (Protection of Vulnerable Groups) scheme. Having a conviction will not necessarily cause a bar to employment.
