We are looking for an exceptional, forward thinking individual to work together with a colleague, to the post of integrated Diabetic/Hot foot support coordinator, you will need to work with the DFC lead, together to ensure the post is covered Monday
- Friday, 52 weeks per year.

This is a key role for GWH and the role requires highly motivated individuals with strong organisational, leadership and clinical skills to undertake this position, to ensure patients receive the right care, at the right time and assist in admission avoidance, deterioration of presentations and the need to initiate appropriate clinical intervention those with diabetic/ hot foot ulcerations. This post is an integrated post and the you will need to work across the clinical pathway supporting patients in primary, community and the acute care settings. You will work under the leadership of the diabetic foot coordinator

The main duties of this non-surgical role are to facilitate and initiate appropriate first line intervention and management for patients and provide expert clinical advice, provide first line treatment and support for people with Diabetic (and acute) foot problems.

You will act as an expert resource for healthcare professionals across the integrated clinical pathway.

The post holder will support the delivery of a comprehensive multi-disciplinary integrated service that is holistic in its approach; evidence based and meets the individual needs of those individuals with complex diabetes related foot complications

The post holder must be able to clearly demonstrate specialist knowledge and skills at an advanced level and responsibilities will include facilitating the MDT with initial assessment and management of acute and deteriorating foot complications. The post holders will demonstrate critical thinking in the decision making process as they deliver a professional approach to the provision of high quality care.

The post holder(s) will need to be able to evidence any improvement and use this evidence to develop the DFC and support the integrated diabetic foot pathway.

The aim of the Diabetic/hot foot coordinator is to support patients and health care colleagues to manage their patients locally without the need to escalate to secondary care for clinical intervention for foot ulcerations.
You will ensure patients receive the right care, at the right time and assist in admission avoidance and deterioration of presentation.
Where patients have deteriorated and need increased care, the role will be to support patients in the journey through the acute pathway, initiating care and ensuring the patient discharge is managed and planned with the patient and our community and primary care colleagues. Managing those with diabetic/acute foot disease is complex and you will be required to facilitate care between all providers across the pathway.

The role will need to ensure there are robust and appropriate key outcome measures recorded for the pathway and the patient.
The post holder must be able to clearly demonstrate specialist knowledge and skills at an advanced level.
They will practice autonomously and also within a defined multidisciplinary team in accordance with local
guidelines and protocols. Responsibilities will include supporting and facilitating the Diabetic foot MDT with
initial history taking, clinical assessment, diagnosis, treatment and evaluation of care for patients. The post
holder will demonstrate critical thinking in the decision making process as they deliver a professional
approach to the provision of high quality care.
