Responsible to: Service Manager

**Purpose of the post**

The wellbeing practitioner will co facilitate psychological education groups alongside assistant psychologists. The duration of the groups will be 1 hour with an additional 30 minutes to complete any outstanding administrative tasks required. There will also be an additional 30 minutes included to allow for travel time

The role will play a key function will be to support approximately clients on the waiting list for support from psychological therapies team

**Main duties and responsibilities**
- Co-facilitate psychological education groups alongside assistant psychologists.
- Provide support to individuals in group settings.
- Have a good understanding of support services across the district and signpost/complete onward referrals for clients to relevant services for additional support.
- Complete individual assessments and outcome measures with clients.
- Develop client-centred support plans to help individuals stay well.
- Record information electronically and maintain up to date client records.
- Work effectively with co-workers
- Motivate and encourage clients to take steps to achieve positive change in their lives
- Accept and work within the organisations policies and procedures and assist in promoting the organisations aims and objectives.
- Have an understanding of and commitment to equal opportunities and diversity.
- Commit to training and self-development and to undertake any training required to carry out the duties of the post.

**Service development**
- Create partnerships with other local organisations to extend the organisation’s service delivery outreach and to help people with mental health issues find clubs, activities, and groups that may be able to support them

**Organisational**
- Complete the necessary paperwork and forms for service delivery at all times

(induction forms, care plans, outcomes forms.)
- Monitor and record positive outcomes of those attending appointments and encourage referrals into employment, training and education.
- Be familiar with the Mind in Bradford ‘Code of Conduct’ and to ensure that it is followed at all times both by staff, volunteers and clients
- Participate in individual and group supervision and internal and external staff development and training
- Attend and input to regular team meetings
- Ensure understanding of and compliance with all Mind in Bradford policies and procedures.
