**Post: Wellbeing Practitioner - SMILE**
**Purpose of the post**
This role is required to deliver high-quality mental health care and support in face-to-face and telephone Settings. Our work goes beyond mental health to address various issues impacting individuals' well-being, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement. You will complete 1- 1 Support for Adults facing mild to moderate mental health challenges. Your role entails actively listening to clients, offering empathetic support, and expertly guiding them towards additional local support services. Each session is one hour, for 6- 8 weeks so building a good rapport with the client is key to the success of the interventions.

& Craven, empowering them to enhance their mental well-being and stay on the path to wellness.

**Key duties**
- Offer personalised one-to-one support, whether over the phone or in person, to clients from diverse backgrounds dealing with various mental health challenges.
- Possess in-depth knowledge of the multitude of support services available throughout the district. Skilfully direct clients to relevant resources and services to amplify their support network.
- Offer support around various issues impacting individuals' well-being, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement.
- Conduct individual assessments and leverage outcome measures to create customised support plans, enabling individuals to proactively manage their well-being.
- Maintain accurate and up-to-date client records electronically, ensuring efficient information management.
- Work seamlessly alongside dedicated colleagues to collectively drive positive change.
- Motivate and guide clients towards taking meaningful steps to transform their lives for the better.
- Uphold and adhere to organisational policies and procedures, actively contributing to the advancement of the organisation's goals and objectives.

**Person Specification**
**Knowledge**
- A minimum of 5 GCSE’s (or equivalent) grades A-C
- Understanding of the key issues within mental health, and the potential challenges that people may face
- Knowledge of other relevant health & social care organisations across the Bradford District &
Craven
- Knowledge of risk management and health and safety issues
- Knowledge of the benefit system with some housing background
- Understanding of safeguarding and good practice
- Understanding of confidentiality and data protection
**Experience**
- Experience of working with young people or adults in a health, social care or any other relevant setting (this can include voluntary experience)
- Experience of supporting people in a clear, meaningful and non-judgemental way either via
- Telephone or in person (this can include voluntary experience)
- Experienced and confident in managing challenging situations, and recognising and responding to safeguarding concerns (this can include voluntary experience)
- Experienced and confident in social support
**Skills**
- Excellent proven communication and organisational skills, including an ability to encourage and engage people in ways to make positive changes in their lives.
- Good understanding IT skills, including use of Microsoft Packages
- Ability to work as part of a team and work alone and use own initiative
- Ability to gather relevant information to support and assess risks
