- An exciting opportunity has arisen to join the Integrated Discharge Team based between the Rotherham Foundation Trust Hospital and the Community Hub at Woodside as a Discharge Liaison Officer.
- The team is a multidisciplinary team of Social Care and Nursing staff who support with adult complex discharges from the hospital and non-acute bed bases in Rotherham, including Intermediate Care and Discharge to Assess pathway. The role will involve being based in the team office either in the hospital or the clinical hub based at Woodside, where the team have continued to work and support the hospital discharges in a safe and timely manner. The team have close links with Rotherham Metropolitan Borough Council, housing and NHS partners, alongside Reablement Service, Continuing Healthcare, Integrated Rapid Response, and the Assistive Technology Service.
- The Discharge Liaison Officer supports the Team Managers, social work and nursing staff to ensure patients are discharged in a timely manner. We work very closely with medical teams on the wards and follow a ‘home first’ approach to ensure where possible patients can return back to their own homes and can maximise their independence.
- The Discharge Liaison Officer (DLO) plays a vital role in the team and will be the link between the staff on the wards, members of the team, and exterior partners such as home care agencies and residential homes. The DLO will also be able to support service users and their families/carers with any queries and advice and ensure that the residents of Rotherham feel supported and informed about their discharge from hospital. The role involves using computer systems including Liquid Logic and Meditech and full training is available on these systems.
- The post holder will support the team and health colleagues to facilitate safe and timely discharges from hospital and play a vital role in supporting the Community Hub to support hospital avoidance. The focus will also be to co-ordinate, collate and track information in relation to progress of assessment and discharge planning in order to report information to Team Managers for Delayed Transfer of Care reporting.
- Key Duties/Areas of Responsibility
- Work as part of a specialist team managing a caseload and supporting assessment of need, negotiate care packages and undertake reviews with consideration for budgetary constraints and in accordance with legislation and council policies.
- Carry out the role of key worker for individual service users, providing assistance and guidance in areas of personal care, daily living, health, social integration and emotional support.
- Ensure implementation of the individual’s care plans, support plans, risk assessments and monitor the effectiveness of these plans and take any appropriate follow-up action in conjunction with the service user, carers and other professional as appropriate.
- Work in partnership with others to ensure a consistent and planned care package is delivered.
- Ensure that parents/carers, service users receive appropriate information and are aware of their rights under appropriate legislation so they can make informed decisions.
- Encourage choice in daily life and promote self-advocacy and/or advocate on behalf of the service user.
- Produce and maintain up to date accurate records to an appropriate standard as required by policy and good practice by using appropriate IT equipment and other methods
- Provide support in a variety of settings i.e. outreach, residential, the Community Hub
- Develop strong working relationships with services, agencies and providers to contribute to effective multi-agency working
