Band 5 to Band 6 Developmental Practitioner Post within the Adult Mental Health Hospital Liaison Team.

Location - The Woodlands Unit, The Rotherham NHS Foundation Trust Site

We are seeking an enthusiastic Band 5 Mental Health Practitioner to join our Adult Hospital Liaison Team.

Your role will be to provide Urgent and Emergency response to Mental Health Referrals from The Rotherham NHS Foundation Trust (TRFT) with whom we work in close partnership. You will undertake in collaboration and under supervision bio-psychosocial assessments, risk assessments and contribute to providing safe and supported discharge plans. You will work closely with TRFT staff members to develop their knowledge of Mental Health - including involvement in delivering training and supporting TRFT to develop positive working practices which put the patient at the centre of their own care and treatment thus promoting parity of esteem.

You will work towards becoming a knowledgeable and a competent practitioner with excellent assessment and risk assessment skills, the ability to work well under pressure and demonstrate the confidence and ability to work autonomously and as a member of the wider MDT. You will have experience of working with adults with complex mental health needs and have a willingness to continue your professional development.

The team works over a 24/7 period, and this involves a flexible duty rota covering this period. However, the development practitioner post will cover core working hours Monday to Friday and occasional weekend shift work.
