**Job Summary**

This is a varied role supporting patients and their carers in hospital, facilitating discharge and supporting them once home. It requires flexibility, creativity and working as part of a multidisciplinary team.

Hospital Frailty Support Workers are expected to support patients on the ward (including the accident and emergency department) and with their discharges (there may not be a Carer involved). This role is a creative one but also one that needs a holistic thinking approach to what will help that person to thrive during their stay but also once home - and the pathway to getting there. There could be an element of engaging the patient in meaningful activities that aid cognition (doll therapy, games, letter writing, reading, crafts, reminiscence etc).

There will be an expectation to support unpaid Carers of patients, providing Carers Assessments, advice, information, and emotional support (including around end of life). Hospital Support Workers will be liaising with professionals, to enable the sustainment of the caring role/discharge, reduce the impact of caring on Carer’s health and wellbeing and to prevent crisis. Hospital Frailty Support Workers will be managing a very varied caseload. The role is to be carried out mainly within the hospital setting. However, it will include working in Carers or patients’ homes and via the telephone.

The post requires an individual who has an understanding of dementia, issues that come with frailty, patients and Carers needs and issues. Previous experience of being in a caring role, working in a hospital, health, social care or voluntary sector setting, is desirable. However, training and support will be provided. As this is in a hospital setting, you are likely to be supporting people who may have Covid.

By ‘Carers’ we are referring to informal Carers who look after a friend or family member with a disability or long-term illness out of a sense of duty and compassion as opposed to a care worker who is employed in their role.

An enhanced DBS check is required for this role.

**Job Dimensions**- To manage a diverse, complex, and fluctuating caseload of Patients and Carers. It is the worker’s responsibility to maintain this caseload at a manageable level (with line management support). To work to action plans with identified timescales and targets. Whilst supporting people it is important to take an empowering and preventative approach to promote independence and improve the sustainability of the situation.
- To undertake developmental tasks and Carer/service awareness-raising as required. This may include being involved with fund-raising; however, this will be discussed with the worker on a case-by-case basis.
- To maintain accurate written and computer records in accordance with local policies and data protection and confidentiality legislation.
- To work as part of a team, participating in team meetings, team building, training and activities.
- To prepare for and participate in regular supervision meetings and to seek additional supervision as required.
- To demonstrate awareness/understanding of equal opportunities and other people’s behavioural, physical, social and welfare needs.
- To ensure that reasonable care is taken at all times for the health, safety and welfare of yourself and other persons, and to comply with the policies and procedures relating to health and safety and safeguarding within the organisation.
- To provide cover for colleagues at Northampton and Kettering General Hospitals.
- To carry out any other duties which fall within the broad spirit, scope and purpose of this job description and which are commensurate with the grade of the post.
