Receptionist /patient Care Coordinator Full Time 9

Monday to Friday Full time 8.30 to 5.00pm (37.5hrs)
Patient Care Co-Ordinator will be required to deal with patients and, if appropriate, their carer(s), before or after the patient's consultation with a clinician or other healthcare professional.
Patient Care Co-Ordinator role requires them to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN.
There may be, on occasion, a requirement to carry out other tasks
- this will be dependent upon factors such as workload and staffing levels)
- answering incoming calls and arranging appointments
- actioning and completing patient referrals
- ensuing that electronic patient communication systems are monitored and issues are resolved in a timely fashion
- dealing with patient's queries and discussing with GPs as required
- dealing with patient referrals
- typing out GP letters, ensuring appropriate layout and the correct template
- receiving incoming post, opening, date stamping and scanning to appropriate places
- receiving and inputting details of new patient registrations
- requesting prescription issues
- assisting with any other admin duties as required (Audits and reporting)
- handling incoming calls
- asking for consent for SMS text messaging
- promoting the use of online services
- actioning patient tasks
- accepting and checking new patient tasks in a timely fashion according to due dates
- providing a friendly, welcoming and professional service to all patients and visitors to the practice
- resolving patient queries
- assisting with any other duties as requested
- to help people to manage their needs by answering their queries and supporting them in making appointments
- to support people to access appropriate benefits where eligible as well as taking up employment and training
Person Specification
- Qualifications
- Educated to GCSE level or equivalent
Experience
- Experience of working in a health care setting
Skills - Excellent communication skills (written and oral) - Good IT skills - Clear, polite telephone manner - Good knowledge of MS Office and Outlook EMIS Web -Effective time management (planning and organising) -Ability to work as a team member and autonomously
- Good interpersonal skills - Problem solving and analytical skills - Ability to follow policy and procedure

**Salary**: £19,000.00-£22,000.00 per year
