PRIMARY PURPOSE OF THE POST The primary purpose of this post is to work as part of Leeds Mental Wellbeing Service (LMWS) Inclusion Team and with key partner organisations to reduce health inequalities and improve mental health and wellbeing of diverse and disadvantaged communities at risk of poor mental health across Leeds. There are four roles which will be based within Talking Therapies and Primary Care Mental Health teams within LMWS. Each Inclusion coordinator will be responsible for 5 primary care networks within Leeds. There are four main elements of this role, which are: To support the Strategic Equalities, Lead (SEL) and the service to implement, contribute and review the actions outlined in the LMWS Health Equity action plan.

To raise the profile and provide a range of awareness sessions around Health Inequalities to all employees of LMWS based on service need. To gather and analyse data collected to identify priority areas within the service in conjunction with the Strategic Equalities Lead. To guide and support the work that Mental Health Support Workers carry out within their dedicated time in relation to Health Inequalities. These 4 objectives will be carried out by using asset-based community development approaches to focus on improving protective factors and reducing risk factors associated with poor mental health and wellbeing through the delivery of targeted work with the wider workforce.

1.4 Facilitate working groups that arise from the Health Equity Steering Group based on the outcomes set out in the Health Equity Plan. 1.5 Engage with frontline staff to ensure feedback on any barriers or challenges are heard and escalated to management or most appropriate avenue. 1.6 To support the work of the coproduction network to ensure that all voices are included in service delivery/design in relation to addressing health inequalities within the allocated Primary Care Networks. 1.7 To provide a targeted approach to improve referral/access within communities highlighted with the Health Equity action plan (such as older people, eastern European).

1.8 To build a range of interpreter services to ensure that there are interpreters readily available to avoid longer waiting times. 1.9 In conjunction with SEL and other Inclusion Coordinators, to lead and implement a communication strategy which recognises the diverse communities of Leeds to increase referrals/information about the service. (e.g., Community led videos). 1.10 To provide additional guidance to Mental Health Support Workers, who have dedicated health inequalities time, by supporting them to create an annual action plan and monitor progress.

1.11 To attend team meetings across the service to report on progress and gather information from teams. 1.12 Ensure that all staff are aware of signposting services and information is regularly circulated. 2. Raising the profile of Health Inequalities within LMWS 2.1 To raise the profile and provide a range of awareness sessions of Health Inequalities to all employees of LMWS based on service need.

2.2 To be a key contact for employees within LMWS regarding any health inequality related work and to provide support, guidance and advice. 2.3 To create information through a range of methods to be shared internally and externally raising the profile of the work being carried out within the service in relation to tackling health inequalities. 2.4 To support recruiting managers to ensure that recruitment campaigns and panels are inclusive and accessible. 2.5 Create Health Equity champions across the service and coordinate regular meetings to share best practice.

2.6 To provide regular induction sessions to new staff members highlighting the importance of health equity work carried out in the LMWS partnership and how to get involved. 3. Gathering and Analysing Data 3.1 To assist the Community Health Development Analyst to engage and gather data and community intelligence on the needs of high-risk communities and population and linking this to Leeds City Councils Public Health Priorities. 3.2 In conjunction with the Strategic Equalities Lead to use the data gathered/analysed to agree targets/priorities that: Identify target communities and population for public mental health interventions.

Help to reduce and eliminate inequalities in mental health service provision targeting priority and disadvantaged communities. Promote protective factors that support good mental wellbeing to flourish and reduce risks to positive mental health. Address stigma and discrimination, particularly focusing on cultural stigmas. Support early intervention and access to primary care and relevant community services.

Promote understanding of barriers to accessing services, and how these may be removed or overcome; and Promote understanding of the services provided, increase uptake and improve equality of access from the targeted community groups. 3.3 To develop and agree a work programme comprising of culturally sensitive and appro
